Smelling the fear in the sweat

"Fearful sweat" samples were collected from male volunteers shown horror films.

Later, female volunteers were exposed to "fearful sweat". They then viewed images of faces that morphed from happy to ambiguous to fearful. They were asked to indicate whether the face was happy or fearful by pressing buttons on a computer.

Exposure to the smell of fear biased women toward interpreting facial expressions as more fearful, but only when the facial expressions were ambiguous. It had no effect when the facial emotions were more certain.

This conclusion is consistent with what's been found with processing emotions in both the face and the voice. There, an emotion from one sense modulates how the same emotion is perceived in another sense, especially when the signal to the latter sense is ambiguous.

This is a direct behavioral evidence that human sweat contains emotional meanings.

They also demonstrate that social smells modulate vision in an emotion-specific way."

Smell is a prevalent form of social communication in many animals, but its function in humans is enigmatic. Humans have highly developed senses of sight and hearing. Why do we still need olfaction? Half-brained girl

The right hemisphere in the girl’s brain failed to develop in the womb.

Normally, the left and right fields of vision are processed and mapped by opposite sides of the brain, but scans on the German girl showed that retinal nerve fibres that should go to the right hemisphere of the brain diverted to the left.

Further, the researchers found that within the visual cortex of the left hemisphere, ‘islands’ had been formed within it to specifically deal with, and map out, the left visual field.

This study has revealed the surprising flexibility of the brain. The brain has amazing plasticity but we were quite astonished to see just how well the single hemisphere of the brain in this girl has adapted to compensate for the missing half.

Despite lacking one hemisphere, the girl has normal psychological function and is perfectly capable of living a normal and fulfilling life. She is witty, charming and intelligent.

She only has slight weakness on her left side (hemiparesis) . The girl had a normal developmental and medical history, attending regular school and taking part in activities such as roller-skating.

In other cases, where patients have half of the brain removed (hemispherectomy) , to treat severe epilepsy for example, one field of vision is lost in both eyes - i.e. they see only objects on the left or right side of their vision. In the case of the German girl, her left and right field vision is almost perfect in one eye.

YAWNING

A yawn consists of a stereotyped behavioural pattern that begins with an inspiration associated with marked dilatation of the pharynx. At the peak of inspiration there are associated facial movements and the final part of yawning is passive rapid expiration. During yawning a coordinated sequence of events takes place involving facial, oropharyngeal, tongue, and respiratory muscles, associated with activity in the axial extensor and limb extensor muscles and with autonomic changes characterised by an increased parasympathetic outflow.

Yawning occurs preferentially in conditions of low vigilance (brain stem) and causes transient increases in arousal (brain stem) as indicated by EEG desynchronisation. Patients with brain stem ischaemic stroke present excessive yawning.

The neural structures that control yawning are presumably located in the brain stem near to or within other respiratory and vasomotor centres, especially those that control facial mimicry, mastication, throat movements, respiration, and possibly stretching.

Yawning also involves the hypothalamus and the midbrain as well as the reticular formation of the pons and medulla (brain stem)

•Yawning is considered to be contagious only in humans by some authors (Lehmann 1979; Walusinski 2004), but this view was recently challenged by a study on chimpanzees (Anderson et al. 2004). The latter study used an experimental video-based approach similar to that used for studying contagious yawning in humans (e.g. Provine 1986; Platek et al. 2003).
•The reported contagion effect in 33% of adult chimpanzees compares with 45–60% typically reported for humans.
•Human mirror neuron system is not involve in contagious yawning, which suggests that yawning is not a truly imitative act (Schürmann et al. 2005).

Using human college-age subjects, the present study tested the hypothesis that yawning is facilitated by higher than normal levels of CO2 or lower than normal levels of O2 in the blood

If yawning is a response to heightened blood CO2, the CO2 mixtures (3-5 %) should increase yawning rate and/or duration. If low blood O2 produced yawning, breathing 100% O2 should inhibit yawning.

The CO2/O2 hypothesis was rejected by the results of this research because breathing neither pure O2 nor gases high in CO2 had a significant effect on yawning although both increased breathing rate.
A second study found that exercise sufficient to double breathing rate had no effect on yawning.

The two studies suggest that yawning does not serve a primary respiratory function and that yawning and breathing are triggered by different internal states and are controlled by separate mechanisms.

Yawning is a stereotyped behaviour present in most mammals from rodents to humans and has been described since antiquity. Hippocrates considered yawning to be an exhaustion of the fumes preceding fever. Modern medicine did not pay much attention to it until the 1980s, when, with advances in neuropharmacology, yawning proved to be a valuable tool for the assessing dopaminergic activity and the pharmacological properties of new drugs. However, its precise role in human physiology is still unknown and its mechanisms remain unclear. The paper by Cattaneo et al (see pages 98–100) reports two cases of pathological yawning as the earliest symptom of brain stem infarction which introduces new arguments for locating this neuronal network in the lower brain stem.

Yawning occurs after waking up, before eating, before sleeping, and in passive activities when it is necessary to maintain a certain level of vigilance.It is then followed by an acceleration of the electroencephalographic rhythms. It does not serve a primary respiratory function and it clearly has a non‐verbal communicative status. Nevertheless, it is also a clinical sign in intracranial hypertension, migraine, or iatrogenic side effects of dopaminergic drugs and serotonin reuptake inhibitors. In basal ganglia disorders, yawning is reduced in patients with Parkinson's disease, and occurs more often in patients with Huntington's disease and supranuclear palsy than in controls. In healthy volunteers, apomorphine induces yawning which is also observed at the beginning of the “on” periods in Parkinson's disease.

The anatomical structures known to be implicated in the occurrence and control of yawning are the paraventricular nucleus of the hypothalamus (PVN), the hippocampus, the reticular formation, the neostriatum, and the cranial (V, VII, IX, X, XI, XII), cervical (C1–C4), and dorsal nerves. Yawning is probably a reflex answer of the brain stem reticular formation aimed to increase the cortical level of vigilance. Dopamine and oxytocin are the main neurotransmitters implicated in its modulation. Indeed yawning induces sensory efferents from the terminals of the fifth facial nerve to the reticular formation or the PVN through the spinothalamic and hypothalamic tracts. Stimulation of the dopamine D2 receptors of the PVN activates the oxytocin neurones that project either to the pons (reticular formation, locus coeruleus), to the hippocampus, to the insula, or to the orbitofrontal cortex, leading to the transient feeling of wellbeing that follows yawning. This pathway is modulated by acetylcholine, serotonin, opioid peptides, sexual hormones, and orexin. The paper by Cattaneo et al provides important data on the crucial role of the lower brain stem.

Contagious yawning is an even more intriguing phenomenon. It is triggered by seeing, hearing, or even thinking about someone else yawning. Contagious yawning does not occur in species that do not recognise themselves in mirrors or in infants younger than two years old. The phenomenon has been investigated with functional magnetic resonance imaging, which implicated the precuneus or the posterior cingulate regions, functional regions associated with the identification of self referent information, a primitive form of empathy. Further studies are needed before conclusions can be drawn

There is no question that the new-born baby is awake. Its eyes are wide open, it wriggles and grimaces, and, most important, it cries. But all that is not the same as being conscious, of experiencing pain, seeing red or smelling Mom’s milk.

New-born baby processes complex visual stimuli and attends to sounds and sights, preferentially looking at faces. The infant’s visual acuity permits it to see only blobs. Her sight will improve in months.

Exposure to maternal speech sounds in the womb enables the fetus to pick up statistical regularities so that the newborn can distinguish its mother’s voice and even her language from others.

The Road to Awareness

The thalamo-cortical complex that provides consciousness with its highly elaborate content, begins to be in place between the 24th and 28th week of gestation. Roughly two months later synchrony of the electroencephalographic (EEG) rhythm marks the onset of global neuronal integration. Thus, many of the elements necessary for consciousness are in place by the third trimester.

Third-trimester fetus is almost always in one of two sleep states. Called active and quiet sleep, these states can be distinguished using electroencephalography. Their different EEG signatures go hand in hand with distinct behaviors: breathing, swallowing, licking, and moving the eyes but no large-scale body movements in active sleep; no breathing, no eye movements and tonic muscle activity in quiet sleep. These stages correspond to rapid-eye-movement (REM) and slow-wave sleep common to all mammals. In late gestation the fetus is in one of these two sleep states 95 percent of the time, separated by brief transitions.

Fetus is actively sedated by the low oxygen pressure (equivalent to that at the top of Mount Everest), and neuroinhibitory and sleep-inducing substances produced by the placenta and the fetus itself such as adenosine; two steroidal anesthetics, allopregnanolone and pregnanolone, one potent hormone, prostaglandin D2 and others. Neonatologists conclude that the fetus is asleep while its brain matures.

The dramatic events attending delivery by natural (vaginal) means cause the brain to abruptly wake up. The fetus is forced from the protected, aqueous and warm womb into a hostile, aerial and cold world that assaults its senses with utterly foreign sounds, smells and sights, a highly stressful event.

Massive surge of norepinephrine as well as the release from anesthesia and sedation that occurs when the fetus disconnects from the maternal placenta, arouses the baby so that it can deal with its new circumstances. It draws its first breath, wakes up and begins to experience life.

OBJECTIVE: The purpose of this study was to examine the dissociative disorder comorbidity of borderline personality disorder and its relation to childhood trauma reports in a nonclinical population.

METHOD: In April 2003, 1301 college students were screened for borderline personality disorder using the Structured Clinical Interview for DSM-IV Personality Disorders. The Childhood Trauma Questionnaire and Steinberg's dissociation questionnaires were also administered. During May and June 2003, 80 students with a diagnosis of borderline personality disorder and 111 nonborderline students were evaluated using the Structured Clinical Interview for DSM-IV Dissociative Disorders by an interviewer blind to the diagnosis and scores obtained during the first phase.

RESULTS: The prevalence of borderline personality disorder was 8.5%. A significant majority (72.5%; 58/80) of the borderline personality disorder group had a dissociative disorder, whereas this rate was only 18.0% (20/111) for the comparison group (p < .001). Childhood emotional and sexual abuse, physical neglect, and total childhood trauma scores had significant effect for borderline personality disorder (p < .001, p = .038, p = .044, and p = .003, respectively) , whereas emotional neglect and diminished minimization of childhood trauma had significant effect for dissociative disorder (p = .020 and p = .007, respectively) .

CONCLUSION: A significant proportion of subjects with borderline personality disorder have a comorbid dissociative disorder. Lack of interaction between dissociative disorder and borderline personality disorder diagnoses for any type of childhood trauma contradicts the opinion that both disorders together might be a single disorder. Recognizing highly prevalent but usually neglected Axis I dissociative disorder comorbidity in patients with borderline personality disorder may contribute to conceptual clarification of this spectrum of psychopathology

The purpose of this study was to see if slow deep breathing, a non-pharmacological procedure known to increase parasympathetic nervous system (PNS) activity, would prevent the development of gastric dysrhythmias and symptoms of motion sickness. In conclusion, slow deep breathing in a situation previously demonstrated to provoke tachygastria prevented the development of gastric dysrhythmias and decreased symptoms of motion sickness.

Uygarligin sonu uzerine

ABD'de Kaliforniya'da, 15 yaşındaki bir kıza sokakta 10 kişi birden 2 saat boyunca tecavüz ediyor. Olaya şahit olan 20 kişi ya umursamadan geçip gidiyor, ya onları seyredip tezahüratta bulunuyor (hani su ciglikli Amerikan tezahüratı!) ve tecavüzü cep telefonuna kaydediyorlar, ya da onlar da kervana katılıyorlar. Kimse polisi aramıyor. Haber yakındaki bir eve kadar gelince (hani biri eve girerken "abi sokağın başında iki araba birbirine girmiş, acayip bir şey" gibi bir cümle kurar ya; bu olayda nasıl bir cümle kurulmuş olabilir ki! ), evdekilerden biri polisi aramayı nihayet akıl edebiliyor.

Freud'un önemli bir eseri "uygarlık ve hoşnutsuzlukları" adini taşır. Uygarlığın hoşnutsuzluklarına nevrotikler katlanabilir ve bunun bedelini de nevroz olarak öderler. Nevrotikler sembolleştirme yeteneği olan insanlardır. ABD, uygarlığın hoşnutsuzluklarına artık tahammül edemeyen ve bunu da sembolleştiremeyenlerin giderek arttığı preodipal bir topluma doğru evrilmenin pek çok işaretini taşıyor.

Fenikel 1930'da nevrotiklerin dışında, karakter patolojisi ile gelen hastaların sayısının arttığını ve artık psikanalistlerin bunu dikkate alması gerektiğini yazdı. Kohut'a göre de (1984; s.60) durum Freud'un zamanından farklıdır. Modern insanın psişesi artık Kafka, Proust, Joyce'un tasvir ettikleri gibi çok parçalıdır (multi-fragmented). Freud bunları tedavi kapsamı dışında bırakmıştı. Kernberg bunların içinde daha ağır vakaları "sınır durum" (borderline) olarak tanımladı. Neye sınır? Tabi ki deliliğe. Yani Batı coğrafyası en azından psikiyatrik populasyon olarak bir yüzyıl içinde nevrozdan borderline'a gerileme işaretleri taşıyor. Bu yüzyılın sonunda da psikoza mı gerileyecek? En azından, sokaklarda, psikotik organize olmuş karakter patolojilerinin sayısı mı yükselecek?

Aslında alametler erken belirmeye basladı. Judith Pearson (New York, Masterson Enstitüsü Baskani) New York'ta "schizopath" denen bir hasta tipini ilk kez gözlemlemeye başladıklarını ifade ediyor. "Şizofren"lerden pek "psikopat"lık sadır olmaz, gerçeklik testleri çok bozuk olduğu için kolay yakayı ele verirler; e biraz saflardır yani! "Psikopat"lar ise kanınızı donduracak vahşetleri göz kırpmadan işleyebilirler. Buna karşılık yakayı ele vermezler, filmlerde bu konu çok islenir, polisleri filmin sonuna kadar peşlerinde koştururlar. "Şizopat"lar ise hem şizofrenler gibi kolay ele düşen ama şizofrenlerden farklı olarak dehşetengiz cinayetler isleyebilen "yeni bir insan türü". Sanki insandan hayvana doğru tersine bir evrimin ilk ara formlarından biri gibi. Uygarlığın batışının uğursuz bir alametine benziyor :-) Hani bir kedi bir kuşu kapar da onu yer, ve bu onun için çok doğal olduğundan kalıntıları saklamaya ihtiyaç duymaz ya, bu şizopatlar da böyle; yani saf bir tarafları da var; aynı hayvanı andırıyor. Freud, uygarlığın sonu için ensest yasağının kalkmasını şart koşar. En son duyduğumuz da şu; birlikte aşk yasayan bir baba-kız, insanlardan "kendi aşklarına saygılı olmalarını" istediler. Tabi konu "aşk", "tercih", "saygı" gibi zamanın en kutsal sözcükleriyle formüle edildiğinde, bugün garipsenen bu durumun, fazla uzun olmayan bir zaman dilimi içinde (belki 30-40 sene) kanıksanacağını ve belki de yasal evlenme haklarını alacaklarını tahmin etmek zor olmaz. Ha, "onların aşkından sana ne?" derseniz, Freud'un, bunu uygarlığın sonu olarak görmesi sizi ilgilendirebilir.

Son olarak, "uygarlığın sonu" temasının bazılarını fazla ilgilendirmediğini görürseniz şaşırmayın. Daha ziyade, çevrenizde uygarlıktan "fazlasıyla yorgun" insanlara dikkat edin. Hala bizimle beraberlerse, yorgunluklarının sebebinin vazgeçilebilecek bir alternatif olduğuna dair zihinlerinde bir imge henüz oluşmamış olduğundan olabilir mi acaba?

Elbette bütün bunları nihilist bir hezeyanın ürünleri olarak görme lüksümüz de var. Ama nihilist hezeyanlar genellikle sahibinin hayatini da kapsayacak şekilde kısa bir süre içinde gerçekleşeceğine inanılan hezeyanlardır. Burada ise bir-iki yüzyıl içinde insanlığın durumunun kötüleşme "eğilimi" taşıdığına dair bir projeksiyon söz konusu.(2) Pek çok antiütopya bundan daha kötümser senaryolar içerir. İklim için söylenenler malum... İnsanların psişe cephesinde ise durum böyle gözüküyor...Hele bir de Sorokin'in "ölmekte olan uygarlıkların kadınsılaştıkları" tezini aklımıza getirirsek

Piaget nesne suregenligini ikinci yilin sonuna yerlestirmesine ragmen, daha sonra yapilan arastirmalar birinci yilin sonunda epigenetik olarak bu yetinin buyuk bir kisminin ortaya ciktigini gosterdiler.

Permanence (suregenlik) nesnenin "kronik" varligina isaret ederken, constancy (sureklilik) nesnedeki libidinal sureklilige gonderme yapar, ontolojik sureklilige degil.

Genellikle tersi bilinir, cunku sureklilik terimi, nesnenin surekli (permanent) varligini cagristiriyor.  

Permanence'in icinde zaman kavrami ickindir (immanence).

Permanence = per + remain'den olusur. Nesnenin "remain" ettigini durdugunu, kaldigini ifade eder. Remain ise re + manere'den (ikamet etmek) olusur. Re- oneki burada tekrar- tekrar, yani, surekli olmak anlamini katar. En bastaki per- ise anlami pekistirmek icindir (intensifier). Yani bir nesnenin yeryuzunde surekli ikamet edebildigini, bir gorunup bir kaybolamayacagini anlamamiz neredeyse bir yilimizi alir. (hayatimizin ilk yili icinde)

Constancy ise con + stare (durmak, ayakta durmak)'dan gelir. Bastaki con- yine pekistiricidir (birlikte anlami vermez). Saglam durmak (stand firm) anlamina gelir. Bir kisinin cesursa cesur, guclu ise guclu durmasi anlamini verir. Cesaretini yitirmemesi, cesaretten korkakliga degismemesi gibi. Yani degismemek, stabil olmak, neyse o olmak, subutiyet (sabitlik) ... (sozlukte "sabitlik" yazar). Annenin bir cok iyi, ardindan da cok kotu olmamasi anlamindadir (libidinal sureklilik). Nesnelerdeki degiskenligin, labilitenin, duraganliga donusmesi anlamindadir. Yoksa, nesnenin "varliginin sabitligi" (suregenlik, PERMANENCE) anlamina gelmez. Yani nesne sürekliliği değil, afekt sürekliliğidir anlatılmak istenen.

Suregenlik zamani da (sure) kapsadigindan permanence'in bu baglamdaki anlaminin cevirisi icin uygundur.

Constancy ise suregenlikten ziyade duraganlik ile ilgili bir kavram oldugundan ceviride sureklilik kelimesinin kullanilmasi problem dogurabiliyor cunku onun da icinde sure var. Sureklilik "ittisal" karsiligi olarak turetilmisti. Ornegin AB dogru parcasinin BC dogru parcasi ile iliskisini anlatirdi. Bununla da AC arasinda sert sicramalar olmadigini, kopukluklar olmadigini, basamaklanma yerine dogal bir egim (evrim, degisim) oldugunu ifade ederdi. Bu nedenle annenin bir gorunup bir yokolmasindan (impermanence) degil, anne kavraminda kopukluklar olusturacak derecede degiskenlik (labilite) olmamasi (stabilite) anlatilmak istenmektedir.

Sonuc olarak sozluk anlami sabitlik olan constancy kelimesinin psikoloji kitaplarinda ittisal anlamindaki sureklilik ile karsilanmasi pek cok kiside bu kavramlarin hala oturmamasina yol aciyor.


[1] Terim aslen Mahler'e ait olmayip, psikanalitik literatur icinde ilk kez 1952 yilinda Hartmann tarafindan kullanmistir. Hartmann bu deyimle cocugun sevgi nesnesine bagliliginin ihtiyac-doyum salinimindan uzaklasarak kalici ve tutarli icsel bir baga donusmesini kastetmekteydi.Hartmann bunun icin saldirgan ve libidinal durtulerin bir olcude birbirlerini notralize etmelerini sart kosmustur.

Kernberg'in gozlemlere ters dusen bu tutumunda israr etmesi, klasik kurama bagliligindan kaynaklaniyor. Cunku Klasik Kuram, superego gelisimini odipal evrenin sonuna birakmak zorunda oldugundan Kernberg, gelisimi bu takvime uydurmaya calisiyor.

Tedirgin Narsisist

Kernberg, bazi narsisistlerde aşagilik ve guvensizlik duygularinin guclu oldugunu ve asil onemlisi bilincli oldugunu soyluyor (1975; s.229). Degersizlik duygusu oldukca belirgin olan ve ulkemizde de oldukca yaygin bulunan bu narsisist grubu literaturde tedirgin, kirilgan, ince derili (Rosenfeld, 1987; s.274), gizli (Masterson, 1993; s.20) narsisist gibi farkli isimlerle anilir.

Bu hastalar, buyuklenmeci (teshirci) narsisistlerin tersine, terapiye dogrudan degersizlik duygulari ile de gelebilirler. Vamik hocanin soyledigi gibi, onlar pastanin guzel tarafini degil de, sirke dokulmus kismini insanlara gosterirler. Bunlarin bir bolumu mazokistik narsisist grubuna girerler.

Narsisistik organizasyon kiside tahammul edilemez duzeyde olan degersizlik ve guvensizlik duygularina yonelik olarak gelisir, ve "omnipotan nesne" (idealizasyon) ve "grandiyoz kendilik" (teshircilik) tasarimlarindan olusan bir savunma birimi olarak ortaya cikar. Tedirgin narsisistler, bu savunma biriminin "grandiyoz kendilik" tasarimini aktifleyen teshirci narsisistlerden farkli olarak, "omnipotent nesne" tasarimini daha fazla aktiflerler. Tedavide ise, yorumlarla, hastanin kendi orjinal kisiligi sandigi seyin aslinda bu savunma birimini yaşayan bir sahte kendilik oldugu ve bu savunmanin "neye karsi" oldugu gosterilmeye calisilir.

Tedirgin narsisistlerde savunmaci birim bir bicimde "defektif"tir. Yani kisi ile dis dunya arasinda, hatta daha da onemlisi gercek kendiligi ile fenomenal beni arasinda bir perde olusturmakta basarisiz olmustur. Teshircilerde ise gercek kendilik, hasmetli duvarlarin arkasinda iyi korunmustur, dis gozlerden gizlenmistir, aslinda asil onemlisi kisinin kendisinden saklanmistir. Teshirciler, ilerleyen yaşla birlikte giderek buyuyen bir bosluk duygusundan yakinirlar. Iceriye hapsettikleri ve gun isigindan mahrum ettikleri gercek kendiligin iniltileri, hayatin ikinci yarisinda o kalin duvarlarda yankilanarak sesini duyurmaya baslar. Teshirci saskindir. Butun omrunu dis dunyasinin, goruntusunun imarina harcamis, hem kendisini hem de diger insanlari kandirmayi basarmis, ancak tehdit hic beklemedigi bir yerden, iceriden gelmeye baslamistir.

Tedirgin narsisistler ise guvensizlik ve degersizlik duygulariyla basetmeye calisarak gunlerini gecirirler. Sosyal fobik ve sizoid gorunurler. Zaman zaman arka planda duran savunmaci birim onlarda da aktiflenebilir ve degersizlik duygulari büyüklenmeci ve omnipotent fantezilerle yer değiştirir. Bu hastalar narsisist olduklarini kabullenmekte gucluk cekerler. Bilinçdışı omnipotan ve büyüklenmeci fantezileri ancak terapide bir süre sonra ortaya cikar.

Bu kişiler ya ortamini bulunca "tedirgin bir teşhircilik" sergilerler, cogu zaman da onlari teshircilik sergilerken gormek olanaksizdir. Cocukluktaki buyuklenmeci hayallerinin yetiskinlikte de uzun sure devam etmesi ile bu grandiyozitenin izleri bulunabilir.

Bu hastalar hayallerindeki buyuklenmeciligi giderek daha da artan idealizasyon savunmasına donustururler. Cunku grandiyozite, eger kaynasma varsa, bir baskasinda da yasanabilir (Kohut'un "kendilik nesnesi", Masterson'in "omnipotan bir isimadan pay alma"). Unlu bir holdingin koruma gorevlisi tarafindan kontrol edilirken veya unlu bir sanatcinin menajeri ile muhatap olurken, onun kibrinin kaynagi hakkinda dusunursunuz. Kaynastigi nesnenin şaşaasindan, sorgulanamaz ve hakedilmis bir pay alinmistir. Bu biraz da gunese nazaran ayin ukalaligina benzer (basking in the glow of an omnipotent object). Bu kişiler büyüklenmeci benliklerini aynalatmak yerine (cunku cocuklukta bu umitlerini yitirmislerdir) , ötekini idealleştirirler. Bu suretle onun "yüceliğine ve ihtişamına sığınırlar".

Tedirgin narsisistlerde paranoid, hipokondriyak ve şizoid özellikler, teshirci narsisiste gore daha baskindir.

Tedirgin narsisistin ayirtedici ozelligi, kendiliklerine dair olumsuz temsil ve hislerinin bilince cok yakın ve cogu kez de bilinc duzeyinde olmasidir. Nedense narsisistik organizasyonlari bunlari hissetmekten alikoyamamistir. Ilk 18 ayda, kaynasmanin sona ermesini engelleyecek hatalar yapan birincil bakici, daha sonraki donemde de cocugun kabarmalarini aynalamamistir. Teshirci narsistler ise, kabarmalari (sempatik ucuslari) guvenli bir inisle (parasempatik) sonlanmadigindan, hala havadadirlar. Büyüklenmecilik ve benmerkezciliklerin i tedirgin narsisistten farkli olarak cüretkâr ve açıkça ifade ederler. Bunlarin spektral ucuna Gabbard (1989) "sinir tanimaz" narsisist diyor.

Tedirgin narsisistlerin bir bolumu mahcup, çekingen ve içedönük görünseler de kendi iclerinde asikar grandiyoz emeller beslerler, ancak "akillica" davranarak acik teshircilik yapmaz, "caktirmdan" uzun vadede kendisini basarili kilacak projelere yonelirler. Diger bir bolum narsistlerde ise, ya savunmaci birim "daha da defektif" oldugu icin veya cok arkaik olup guncel savunmalar uretmekten uzak oldugu icin, bu narsisistlerin "gundem"inde hicbir narsisistik tedarik barinamaz. Gerceklestirdikleri hiçbir basariyi ustlenemezler. Bu basarilari, uzerlerinde kalamaz, kayip yere duserler. Bunlarin icsel yapisi terapiste guvenmeye karar verirse eger, intrapsisik yapinin sizoid oldugu ortaya cikabilir. Diger durumlarda ise teshis ortada kalir. Cunku hastanin belirsiz transferansini terapist anlayamamistir. Hasta intrapsisik yapisini acamadigi icin terapist de ne şizoid dilemmayi ne de narsisistik incinmeyi yorumlayabilir. Bu tur hastalarin sizoid olma ihtimalleri yuksek. Kernberg bu tur hastalari tedavi edilemeyecek olan % 40'in bir bolumunu olusturuyor. Klasik yorumlamalar ve yuzlestirmeler sizoid hastalari korkutur. Sizoid hasta karsisinda terapistler daha pasif bir konumda kalirlar. Bu tutum hastaya cok ihtiyaci oldugu hareket alanini saglamakla birlikte, tedavi suresini uzatir. Terapist bu pasif tutumun kendi karsi aktariminin eyleme vurumu olup olmadigi konusunda icgoruye sahip olmalidir. En onemli ipucu, Kernberg'in beklediginin aksine, bilincdisi omnipotan hezeyanlarin ve narsisistik buyuklenmeci fantezilerin yuzeye cikmasi (1975; s.229) bir yana, seanslar ilerledikce aynalanma, büyüklenmecilik, idealizasyon gibi temalarin giderek azalmasidir.
